2021 Carillon Summer Concert Series Pre-Registration Form
Per University of Michigan policy, we are required to make and keep a record of all in-person attendees and staff for 21 days following the concert. This information will be used for contact tracing if confirmed COVID-19 cases become associated with the gathering or activity.
